چکیده

V ENTRICULAR extrasystoles can occur anywhere in the cardiac cycle. When they occur toward the end of diastole they may be synchronous with the next conducted impulse resulting in a fusion complex. We report a case of left bundle-branch block associated with end-diastolic ventricular extra-systoles that occur in bigeminal rhythm; this results in fusion complexes that apparently normalize the bundle-braneh block pattern. The resulting arrhythmia simulates 2 :1 left bundle-branch block.

A study in intracardiac conduction. 'Normalization' of bundle-branch block by fusion with ventricular escape beats.

A ventricular fusion beat reflects the simultaneous activation of the ventricles by two impulses usually a sinus impulse and an ectopic ventricular impulse each activating part of the ventricles. The resulting QRS complex has a configuration that is in between that of the 'pure' uncomplicated sinus beat, and the 'pure' uncomplicated ectopic ventricular beat. If, however, the sinus beat is condu...

Images and Case Reports in Arrhythmia and Electrophysiology Premature Beats and Unexpected Heart Block An Unusual Mechanism Confirmed by Ablation

A 23-year-old, otherwise healthy woman presented with recurrent skipped and rapid sustained heart beats. Her physical examination and echocardiogram were normal. The presenting ECG in Figure 1 shows sinus rhythm, a nonconducted sinus p wave, and premature complexes. Both left bundle-branch block (5 and 6 complexes) and right bundlebranch block (10 complex) morphologies are present. The 5 and 10...

Patterns of ventricular contraction in patients with conduction abnormality studied by radionuclide angiocardiography.

The phase image generated by radionuclide angiocardiography illustrates the regional timing of ventricular wall motion. In this study the phase image was used to investigate the patterns of ventricular contraction in 103 subjects with either normal hearts or a conduction abnormality. In 38 normal subjects the right ventricle contracted on average 7 ms after the left, and the last region to cont...

Alternating bundle branch block during atrial bigeminy.

Alternate patterns of ventricular excitation may occur with any type of supraventricular bigeminy. It is a physiologic phenomenon that depends on the rate dependency of bundle-branch refractoriness and the linking phenomenon related to concealed retrograde penetration of aberrantly conducted beats into their own blocked bundle branch [1–3]. An alternate pattern of ventricular activation can onl...
